Facelift

Dr. Jaibaji performs face lift which is also known as rhytidectomy, a facelift is a surgical procedure to improve visible signs of aging in the face and neck.

A facelift can only be performed surgically; non-surgical rejuvenation treatments cannot achieve the same results, but may help delay the time at which a facelift becomes appropriate and complement the results of surgery.

As a restorative surgery, a facelift does not change your fundamental appearance and cannot stop the aging process.

Rejuvenation procedures typically performed in conjunction with a facelift are brow lift, to correct a sagging or deeply furrowed brow, and eyelid surgery to rejuvenate aging eyes

Who is a candidate for a facelift?
If you are bothered by the signs of aging in your face, a facelift may be right for you. The visible signs of aging in the face and neck, are:

  • Sagging in the midface
  • Deep creases below the lower eyelids
  • Deep creases along the nose extending to the corner of the mouth
  • Fat that has fallen or is displaced
  • Loss of muscle tone in the lower face may create jowls
  • Loose skin and excess fatty deposits under the chin and jaw can make even a person of normal weight appear to have a double chin

What to expect during your consultation

The success and safety of your facelift depends very much on your complete candidness during your consultation. Dr. Jaibaji will ask you a number of questions about your health, desires and lifestyle. Be prepared to discuss:

  • Why you want the surgery, your expectations and desired outcome
  • Medical conditions, drug allergies and medical treatments
  • Use of current medications, vitamins, herbal supplements, alcohol, tobacco and drugs

Facelift surgery is a good option for you if:

Preparing for surgery
Prior to surgery, you may be asked to:

  • Get lab testing or a medical evaluation

Your plastic surgeon will also discuss where your procedure will be performed. A facelift may be performed in an accredited office-based surgical center, outpatient or ambulatory surgical center, or a hospital.

You’ll need help

If your facelift is performed on an outpatient basis, be sure to arrange for someone to drive you to and from surgery and to stay with you for at least the first night following surgery.

Procedural Steps

What happens during facelift surgery?

Step 1 – Anesthesia

Medications are administered for your comfort during the surgical procedure. The choices include intravenous sedation and general anesthesia. Your doctor will recommend the best choice for you.
A variety of other procedures can further enhance the outcome of a facelift. They include:

  • Facial implants
  • Soft tissue augmentation to recontour the facial structure
  • Resurfacing techniques to improve the tone and texture of facial skin
  • Wrinkle reduction by injection

Step 2 – The incision

Depending on the degree of change you’d like to see, your facelift choices include a traditional facelift, limited incision facelift or a neck lift.

A traditional facelift incision often begins in the hairline at the temples, continues around the ear and ends in the lower scalp. Fat may be sculpted or redistributed from the face, jowls and neck, and underlying tissue is repositioned, commonly the deeper layers of the face and the muscles are also lifted.

Skin is redraped over the uplifted contours and excess skin is trimmed away. A second incision under the chin may be necessary to further improve an aging neck. Sutures or skin adhesives close the incisions.

Traditional Facelift

An alternative to a traditional facelift uses shorter incisions at the temples, continuing around the ear and possibly within the lower eyelids or under the upper lip.

Limited Incision

Sagging jowls, loose neck skin and fat accumulation under the chin may be corrected with a neck lift. The neck lift incision often begins in front of the ear lobe and wraps around behind the ear ending in the lower scalp.

Step 3 – Closing the incisions

Once healed, the incision lines from a facelift are well concealed within the hairline and in the natural contours of the face and ear.

Important face about the saftey and risks of facelift surgery

The decision to have a facelift is extremely personal and you’ll have to decide if the benefits will achieve your goals and if the risks and potential complications are acceptable.

Dr. Jaibaji will explain in detail the risks associated with surgery. You will be asked to sign consent forms to ensure that you fully understand the procedure you will undergo and any risks and potential complications.

The risks include:

  • Unfavorable scarring
  • Bleeding (hematoma)
  • Infection
  • Poor wound healing
  • Anesthesia risks
  • Correctable hair loss at the incisions
  • Facial nerve injury with weakness
  • Facial asymmetry
  • Skin loss
  • Numbness or other changes in skin sensation
  • Fatty tissue found deep in the skin might die (fat necrosis)
  • Fluid accumulation
  • Pain, which may persist
  • Skin contour irregularities
  • Skin discoloration, sensitivity or swelling
  • Sutures may spontaneously surface through the skin, become visible or produce irritation that require removal
  • Unsatisfactory results may include asymmetry, unsatisfactory surgical scar location, unacceptable visible deformities at the ends of the incisions (It may be necessary to perform additional surgery to improve your results)
  • Deep vein thrombosis, cardiac and pulmonary complications
  • Possibility of revisional surgery

Be sure to ask questions: It’s very important to ask your plastic surgeon questions about your facelift procedure. It’s natural to feel some anxiety, whether it’s excitement for your anticipated new look or a bit of preoperative stress. Don’t be shy about discussing these feelings with your plastic surgeon.

My Recovery

When your procedure is completed, a bandage could gently be placed around your face to minimize swelling and bruising. A thin tube may be present to drain any excess blood or fluid that may collect under the skin.

You will be given specific instructions that may include: how to care for the surgical site, medications to apply or take orally to aid healing and reduce the potential for infection, specific concerns to look for at the surgical site or in overall health, and when to follow up with your plastic surgeon.

The results will be long-lasting

It may take several months for swelling to fully dissipate and up to 6 months for incision lines to mature.

  • Hematoma: Blood pooling beneath the skin.

Endoscopic Brow Lift

In addition to facelift surgery, we also offer exceptional endoscopic brow lift surgery for patients who require less extensive facial rejuvenation. Endoscopic brow lift surgery is effective at lifting the brow line, thus reducing the prominence of wrinkles and creases that develop on the forehead as a result of the natural aging process. Endoscopic brow lift surgery is less invasive than a full facelift, but the procedure can reverse the visible effects of aging just as dramatically, restoring an alert and youthful appearance with fewer incisions. The result is minimal scarring and a faster recovery.

The Brow Lift Procedure

During endoscopic brow lift surgery, tiny incisions are made just above the hairline. Dr. Jaibaji uses an endoscope, a tiny camera with a light, to guide himself through the entire process. He removes excess tissue and fat from the forehead and then slightly lifts the skin to smooth out wrinkles and elevate the eyebrows. Next, the skin is fastened in its new place. Endoscopic brow lift surgery can be combined with a neck lift, rhinoplasty, or another procedure to create a completely rejuvenated appearance.

Recovery from Endoscopic Brow Lift

Many patients choose to undergo endoscopic brow lift surgery because it requires less recovery time and leaves very little scarring in comparison to traditional brow lift or facelift surgery. Some pain, swelling, and numbness may be experienced for a few days, but medication can be prescribed to treat any discomfort. Blepharoplasty

Often a sign of aging, droopy eyelids can make individuals appear falsely tired, melancholy, or older than they actually are. Blepharoplasty, commonly known as eyelid surgery, can restore an alert, youthful, and cheerful appearance to droopy eyelids and eliminate fine lines and wrinkles that develop around the eyes as a result of aging. Blepharoplasty can be performed on both the upper and lower eyelids and in fact is usually conducted on both at the same time. Depending on which version or versions are chosen, eyelid surgery can eliminate extra skin and fat around the eyes that hinders vision and get rid of bags that develop under the eyes.

The Blepharoplasty Procedure

During the blepharoplasty procedure, small incisions are made in the crease of the upper eyelid or along the lash line of the lower eyelid. The incisions are strategically placed to minimize the visibility of scarring after surgery. Next, excess skin and fat are trimmed away. Finally, the incisions are closed with fine sutures. Blepharoplasty surgery typically takes a few hours and can be combined with a facelift, rhinoplasty, or another plastic surgery procedure for the ultimate makeover.

Recovery from Blepharoplasty

After eyelid surgery, patients should expect some redness, bruising, swelling, and soreness around the incision areas, which should subside in a couple weeks. Other side effects may include dry or teary eyes, as well as some sensitivity to light. Dr. Jaibaji can prescribe medication to reduce discomfort or pain. He will also inform the patient of special postoperative instructions that can help expedite the healing process. Rhinoplasty

Common reason patients seek to see Dr. Jaibaji for nose surgery is:

  • A hump on the bridge of the nose
  • A wide bridge and /or a wide nasal tip
  • Being able to see the nostrils too much
  • Large of wide nostrils
  • A crooked nose
  • A "ski-jump" nose
  • "high" or flared nostrils

Before Surgery

Dr. Jaibaji will evaluate the patient before surgery. His evaluation includes:

  • Your general health and any medical problems
  • Previous nasal injury, surgery or allergy
  • How you breathe through your nose, and whether or not this needs to be addressed at the time of surgery; i.e. performing septal surgery
  • The size and shape of each part of your nose (tip, bridge, nostrils, etc.) in relation to the other parts of your nose; i.e. is it proportional
  • The size and shape of your nose in relation to the rest of your facial features (e. g. your chin, lower jaw, etc)

The Surgery

Rhinoplasty is usually performed under general anesthesia as a same day surgery. Depending on the desired result and the complexity of your specific problem, the surgery may take anywhere from one to three hours.

The details of the specific type of nose surgery suited to your specific aesthetic concerns and functional requirements and the placement of the incisions is determined and discussed at length by Dr. Jaibaji during the first consultation. There are risks and complications with all surgical procedures such as bleeding and infection. These as well as complications specific to rhinoplasty will be discussed at the time of your consultation.

After Surgery

  • 1st post-operative visit within 48 hours after surgery; to check for bleeding, level of discomfort, to answer any immediate concerns.
  • 1 week after surgery; splint is removed, sutures (if external sutures are required) are removed, in spite of swelling, difference in appearance of nose if often immediately evident, resume normal light daily activities
  • 10 - 14 days after surgery: return to work, most of the bruising is usually resolved, swelling reduced, vigorous activity (exercise, heavy lifting) are usually restricted till about three weeks after surgery. Contact sports can be resumed at 6-8 weeks after surgery.
  • 6- 8 weeks after surgery: swelling significantly reduced, resume normal activities, exercising etc.
  • 3 - 6 months; most of swelling gone
  • 6 months - 1 year: remainder of swelling disappears.

Dorsal Hump

The dorsum of the nose refers to the top of the nose. Normally, it is straight or has a gentle convexity towards a more projecting nasal tip.  The dorsum of the nose may be satisfactory when viewed from the front, but may show too much projection when viewed from the side.

The nose, as the central feature of the face, has a significant effect on facial aesthetic look. The nasal dorsum determines the character of the nose. When the dorsum of the nose is concave or has a bump or a hump, this may interfere with the desired profile.

In addition to the height and configuration of the nasal dorsum, the projection and shape of the nasal tip affects the nasal profile, and the contribution of both of these factors must be considered. Moreover, the quality of the skin is especially important in nose surgery. Thick skin may not allow the altered anatomy to show through, whereas very thin skin may show any slight irregularity.

When doing a nose reshaping surgery, a number of surgical techniques are available to reduce the dorsal hump to create better balance to the shape of the nose.

The Wide Tip

Another common concern with which rhinoplasty patients seek Dr. Jaibaji is a wide, 'bulbous' or ';boxy' tip. This is most often heredity and may be partly a result of a thick nasal skin. Most patients seek refinement of the tip, and this must be carried out by the surgeon in such a manner as to maintain the natural appearance of the nose. A tip that is too pointy or small, particularly in proportion to the rest of the nose may result in an unnatural, "operated on" appearance.

In patients with thin nasal skin, a wide tip is a consequence of large or splayed cartilages that form the tip.

In order to increase the precision of reshaping and resizing the tip, Dr. Jaibaji uses an “open approach� rhinoplasty technique. This method facilitates direct visualization of the nasal cartilages. It also increases the predictability of the final surgical result. The open approach also allows for precise suturing of the nasal cartilages to reshape and refine them as well as position them more accurately.

Asymmetry of Nose

A nose may be crooked as a result of heredity, previous surgery, or trauma. It may be crooked in certain portions (tip, bridge, nostrils...) or the entire nose may be asymmetrical.

Regardless of the cause; Dr. Jaibaji will determine what structural problem is causing the asymmetry. It may be a cartilage problem (e.g. the septum), a bone problem, or a combination of both. The specific procedures to address the asymmetry will depend on these structural factors. Often, obstructed nasal breathing accompanies asymmetry of the nose, and these can be corrected using the techniques mentioned below.

The surgery may include cutting and repositioning the nasal bones (osteotomies), reshaping the nasal septum (septoplasty), septal extension, cartilage grafts and a variety of other techniques with which Dr. Jaibaji familiar with.

Neck Lift

Neck lift surgery at our cosmetic surgery practice provides another effective way to improve one's appearance. The surgery involves removing excess skin or fat from under the chin and on the neck and repositioning the remaining tissue to produce a firmer, more elegant neckline and a well-defined jaw contour. Our experienced surgeon, Dr. Moneer Jaibaji, can use a variety of advanced techniques to improve the appearance of the neck, including liposuction, cervicoplasty, and platysmaplasty. These state-of-the-art techniques make neck lift surgery a safe and relatively quick procedure that dramatically enhances aesthetic appearance.

The Neck Lift Procedure

During the neck lift procedure, tiny incisions are made behind the ears or below the chin. Next, liposuction is used to remove excess fat in the neck, producing a slimmer and smoother neck contour. Excess skin is removed from the neck through a process called cervicoplasty. During this process, the doctor lifts the skin and redrapes it farther back to improve its appearance. The skin is then secured in its new position with fine sutures or a special adhesive. Platysmaplasty, a technique that is used to realign or remove specific areas of neck muscle, may also be performed to reduce the amount of "banding" in the neck. Sutures are used to hold the muscles in the optimum place. The entire neck lift procedure takes up to three hours and is often performed in conjunction with a facelift, a brow lift, BOTOX® injections, or another cosmetic treatment for an amazing transformation.

Recovery from Neck Lift Surgery

Recovery from neck lift surgery usually takes at least a week and final results may not be visible for a few months, but for most patients it is well worth it. A slimmer neckline can create an illusion of dramatic weight loss and can even promote a more youthful appearance. Some swelling and bruising are normal after neck lift surgery, but these side effects should disappear within one week. Dr. Jaibaji may recommend that some patients wear compression bandages to facilitate faster healing. Most patients are able to return to work within two weeks of their surgeries. Ear Pinning

Ear pinning, also known as otoplasty, is a procedure that is used to improve the appearance of ears that are large or stick out too far from the head. By pinning the ear so that it lies flatter against the side of the head, the procedure can greatly enhance an individual's appearance and self-esteem. This procedure is common for children between ages four and 14 who are embarrassed about or dissatisfied with the appearance of their ears.

The Ear Pinning Procedure

During ear pinning surgery, Dr. Jaibaji creates an incision behind the ear, allowing him to fold, reshape, or remove cartilage and lay the ear closer to the head. The incisions are then sutured shut and bandages that typically stay in place for approximately one week are applied to the ears. Ear pinning is a relatively safe and effective procedure with a short recovery time. Because of this fact, ear pinning can be performed on children as young as four but is also suited for adults of any age. Ear Repair

Women and men wear ear jewelry to enhance their looks. Since ancient times rings, posts, and other objects have adorned ears. Various parts are pierced to hold these items in place. However, the effect of weight/ gravity and/or trauma can pull through tissue resulting in a split ear lobe. This defect is usually very visible and hard to cover with hair or clothing.

he soft ear lobe is made of skin and fat. Other parts of the ear have cartilage for structural strength. Since the tissue over the cartilage is thin, contour irregularities of the cartilage are easily seen. Reestablishing the delicate shape of the underlying cartilage adds complexity to reconstruction.

The tiny wires that hold jewelry can tear through the tissue when sharply yanked or with enough pressure slowly over time. Larger loops and bigger items are more easily snagged. Small posts are less likely targets but can still cause damage.

Large or heavy items will gradually elongate the hole and eventually will tear through the lobe. Larger masses have more momentum and damage more easily happens during activity. If you cannot live without your heavy jewelry, be careful about quick movements. Lighter smaller adornments are safer.

The Repair

Dr. Jaibaji will remove any skin lining the split to create a raw edge, and will then stitch the ear back together. A z-plasty or flap procedure may be required to close the tear to allow for adequate strength for another earring.

Earlobe repair is an office procedure performed under a local anesthetic, so you'll be able to return home the same day.

Postoperative Care

you will need to wash your wound with soap and water 2-3 times a day. A thin layer of antibiotic ointment is applied twice a day. You may wash your hair the day after surgery.  Expect some drainage from the repair site. Sutures are usually removed after 5-7 days.

The patients can start massaging their scar with cocoa butter 15 minutes a day starting two weeks after surgery. The massaging seems to soften the scar over time.

Wearing an earrings after surgery?

If a suture was placed to recreate the pierced ear; it will be take out after two weeks. Provided the wound has healed and there is no sensitivity; you can wear clippers two weeks after surgery. Regular ear rings can be worn 6 weeks after surgery when the scar is stronger. However, heavy jewelry can still tear the repair and is not recommended.

If the ear-piercing hole is closed up during a split earlobe repair, although the ear can usually be re-pierced after 6 weeks.

Keloid ear

Keloids are overgrowths of scar tissue that follow skin injuries. Keloids may appear after such minor trauma as ear piercing. They can also occur following surgical incisions, traumatic wounds, vaccination sites, burns, chickenpox, acne, or even minor scratches. Dark skinned individuals tend to form keloids more readily than lighter skinned individual.

Most keloids will flatten and become less noticeable over a period of several years. They may become irritated from rubbing on clothing or other forms of friction. Extensive keloids may become binding, limiting mobility. They may cause cosmetic changes and affect the appearance.

Exposure to the sun during the first year of the keloid's formation will cause the keloid to tan darker than surrounding skin. This dark coloration may become permanent.

Keloids usually are not medically dangerous, but they may affect the cosmetic appearance. In some cases, they may spontaneously reduce in size over time. Removal or reduction may not be permanent, and surgical removal may result in a larger keloid scar.
